چکیده

Lumbar disc herniation (LDH) is a very rare clinical entity in the pediatric age group, therefore only a few clinical studies have until now investigated the clinical behavior of pediatric LDH. The natural clinical history, postoperative neurological recovery with radiological follow-up and quality of life related to the disorder continue to be unknown. We prospectively planned and designed two groups of LDH patients scheduled for surgery in this study. The operations were performed by the same surgeon in a randomized fashion. The first group consisted of pediatric patients and the second group of adult patients. Both groups contained 32 cases. Postoperative neurological recovery was assessed using the visual analog scale and Aberdeen Low Back Pain Scale. Postoperative peridural fibrosis was also evaluated in postcontrast magnetic resonance sections. After 3 years of follow-up, all parameters were significantly better in the pediatric LDH group with no recurrence of the disease. The pediatric LDH group was postoperatively better than the adult group both clinically and radiologically. Although LDH seems a catastrophic disease in the pediatric age group, the clinical picture and neurological recovery have a significantly more benign course than in adult patients even in operated cases.

Fragmentectomy versus Conventional Microdiscectomy in Single-Level Lumbar Disc Herniations : Comparison of Clinical Results and Recurrence Rates

OBJECTIVE This retrospective study aimed to compare clinical outcomes in terms of pain relief and recurrence rate between fragmentectomies and conventional microdiscectomies in patients with lumbar disc herniation (LDH). METHODS Between January 2008 and May 2011, a total of 175 patients met the inclusion criteria of this study. The visual analogue scale (VAS) scores of back and radicular pain...

Clinical Outcomes after Microdiscectomy for Recurrent Lumbar Disk Herniation: A Single-Center Study

Background: Revision discectomy is the principal procedure for recurrent lumbar disk herniation (RLDH). The clinicaloutcomes after this procedure are as good as or slightly poorer than those produced by primary discectomy. In thisstudy, the clinical outcomes of patients treated with microsurgical discectomy for RLDH were analyzed. Methods: We examined 179 patients undergoing lumbar microdiscecto...

Randomized clinical trial comparing lumbar percutaneous hydrodiscectomy with lumbar open microdiscectomy for the treatment of lumbar disc protrusions and herniations

OBJECTIVES Hydrodiscectomy is a new technique used for percutaneous spinal discectomy that employs a high-intensity stream of water for herniated disc ablation and tissue aspiration. No previous clinical study has examined the effects of percutaneous hydrodiscectomy. The aim of this study is to evaluate the outcomes of hydrodiscectomy compared to open microdiscectomy regarding pain, function, s...

Comparison the Recurrence Rate of Intervertebral Disc Herniation in Two Surgical Methods of Hemi laminectomy and Partial Laminectomy

Background & Aims: Between 5% and 15% of patients with low back pain suffer from lumbar disc herniation, so intervertebral disc disease is one of the most common causes of low back pain. Surgical intervention for lumbar disc herniation is recommended in patients with severe symptoms and has hopeful results. Although surgery can reduce pain and improve physical function in the short term, its lo...

Extreme Lateral Endoscopic Transforaminal Lumbar Discectomy for Lumbar Disc Herniation in Young Patients

Objective: The herniated lumbar disc (HLD) in young patients is increasing. The endoscopic discectomy has been used as a treatment option, but it is limited in the management of large extruded or migrated disc herniations. Recently, with an application of extreme lateral endoscopic lumbar discectomy (ELELD), it is possible to obtain access to epidural space directly and to remove herniated disc...
